I got one that was max damage req10 from the treasure in the sulfur wastes.
Got a req.10 one from a Ravenous Mandragor in the Alkali Pan. Think that's what the mob was called at least - a mandragor necro.
I got one in the Sulfurous Wastes from a Graven Monolith.
